I don't know. It's weird. I am almost certain though it has to be bacterial or a fungus (and probably the latter.) If it was a parasite my two cleaner wrasses would be hounding him.

Looks a bit like an injury that might be infected or have fungus on it.

I’ve seen bacterial infections that look similar, but I’m no expert. Have you tried the fish disease forums on R2R? There is a lot of helpful expertise hanging out there, even though it’s not always a great experience.

So far the (small) consensus there is Lympho's. I hope they are right.

Polylab Medic says it's useful for external parasites. I am pretty sure this isn't that. It also always sends up a red flag when any company says it's in-tank treatment works on Ich.
