and I am sure you are right. I've been second guessing myself as of late. Last night I was chatting with a couple of guys at Neptune and they mentioned there could have been a battle with another fish since this isn't the first death I've had in less than two weeks. But I did not see any wounds/scars, etc.
 
Found my juvenile angle dead today. No visible scars/wounds. I am just baffled at this point. I've never had 5 fish die in a span of two weeks.

Any recent pics of the live fish you can share? I'd do a big water change, send in an ICP test of current water to Trident (I think they have the fastest turnaround to the west coast), make sure your salinity gauge is calibrated, and put in a bag of carbon. I'd also consider getting a bag of whatever the heavy metal removing media is, whose name I can't recall. I don't know if it helps with iron, but can't hurt.

Are the fish eating? No long stringy poops intestinal parasite sign)?

i didn’t take too many pics of the fish as it was kind bare.

I did do an ICP test and it came back last Friday. Nothing I didn’t already know. Higher phosphates. No heavy metals indicated in the report. See images below.

As far as the string poop, I have not seen any stringy poop to be honest.

No big changes.
Same schedule for the past couple of years. Two months ago I got the plank and been using it to feed the same food/pellets, but also mixed plankton with it. The fish devour it. Of course I also add nori for the tangs and other herbivores.
 
Found my powder blue dead this morning. I don't see any scars/wounds. I'm just speechless.

On top of this, I am leaving on a trip tonight for a week. Will see what's left when I get back.

When I get back, I am going to start setting up another smaller tank. Take all the fish out. Almost drain the tank, and start over. What a #$%^&* mystery this has been.

Have you added any new livestock (fish, inverts, corals) recently?

Velvet can sometimes asphyxiate fish before external symptoms appear since it usually attacks the gills first. While this would be odd that no fish has shown external signs of velvet since this was a two week affair, I supposed it could be possible.

Not before this ordeal started. I added a nem two weekends ago and a couple of frags from folks this past weekend. I dip all my corals and acclimate.... but to your point, this could just be a slow death for most of them.

Assuming you've done a big water change, seems like disease. Maybe try posting on humble fish?

Did the dying start happening right after the nem?

Nothing wonky with your water source? How are the corals doing?

No. The dying started before adding the nem.

As far as the water, not that I am aware of.

To your point it could be a disease of some sort.
 
This somewhat reminds me of what @popper went thru couple months ago when he introduced >2in devil damsel to his tank.
It slowly harassed/killed his big tangs and other fish.

So what’s your plan going forward? I think the most sensible approach might be to go fishless for 65 days or whatever the suggested timeframe is to allow the diseases to die. You can bump temps up to 79or80 to speed the process. This would allow you to sort of reset, choose the fish that mean...

yellow belly Blue Tang 4+ years.
Sohal Tang 6 months
Purple Tang 6 months
Bangi bijama fish 3 month
Anthias 3 months

There are a few others that I am just too depressed to remember.

This could be the case. Though, I don't have a blue devil damsel, but I did get some blue Chromis fish.

As of now, I have a clown tang, a yellow tang, a blond naso tang, chromis, and a yellow head gobi.
